APPENDIX A - Hazardous Materials Label for Containers of Chemical Waste (pdf)

KU-EHS required label to be put on all containers of hazardous chemical waste for which the user is requesting pick-up.

APPENDIX B - EPA Hazardous Waste Characteristics

Any hazardous materials waste exhibiting any of these characteristics meets the definition of an EPA characteristic hazardous waste and must be picked up by EHS for proper disposal.

APPENDIX C - EPA Hazardous Waste Lists

Any hazardous materials waste which contains one of these compounds or matches up to one of these listings meets the definition of an EPA listed hazardous waste and must be picked up by EHS for proper disposal.

APPENDIX D - Other Hazardous Chemicals Lists

These are specific lists of chemical compounds and reagents that exhibit significant health, safety and/or environmental hazards which require them to be carefully collected by the user. These materials must be picked up by EHS for proper disposal.

APPENDIX E - Radioactive Source Summary/Waste Manifest Form

The source summary and waste manifest provides the required Department of Transportation documentation to transport radioactive materials on KU and public highways and for keeping an accurate inventory of sources used in the laboratory.
